    • Garrett’s law was enacted in 2004.  It mandates schools provide parents and guardians with information about meningococcal meningitis and influenza and the vaccines that protect against these diseases. The law was expanded in 2007 to mandate that information also be provided about human papillomavirus (HPV) and the vaccines available to protect against HPV.
    • Health Assessment/Physical Requirements for School Attendance
      • Kindergarten
        • Proof of required school-entry immunizations; and,
        • NC Health Assessment Transmittal Form completed no more than 12 months prior to school entry.  Form: English   Espanol
      • Students, grades 1-12, who are attending public school in North Carolina for the first time:
        • Students without proof of a Health Assessment made within 12 months of school entry must have one completed on this Form.
        • Proof of Health Assessment made no more than 12 months prior to school entry; or,
        • Health Assessment completed on the NC Health Assessment Transmittal Form.
      • The 30-calendar day rule applies to all of the above.
        • The parent/legal guardian is responsible for submitting all required documents (listed above) to the school within 30 calendar days of school entry.  
        • Students who do not meet these statutory requirements are not allowed to attend school.  
        • Students will be re-admitted/allowed to return only when the required documents are received.
        • Note: The count starts with 1st day of attendance as calendar day #1 ... the student has 30 full calendar days to meet immunization and/or health assessment requirements...if not in compliance on the morning of the 31st calendar, the student may not attend school -- and may attend only when the requirements are met/proof provided to the school.
